DuVita Health Insurance Update - New Health Insurance Program
September 24, 2010 - DuVita along with Insured Benefit Plans Inc. and Adroit Mutual Benefits are pleased to announce a New Health Insurance Program that will be available to all active DuVita Associates effective November 1, 2010. The new health plan gives you access to needed healthcare at affordable rates. Many of these plans are guaranteed issue so you cannot be turned down for coverage.
You have a full range of plans available to you. The medical plans include coverage for doctor visits, wellness, hospital, surgical, accidents, critical illness and more. The medical plans also include an insured Rx card, telemedicine provided by AmeriDoc, discount dental & vision plan and a no cost annual adult wellness exam.
You also have access to fully insured stand alone plans such as Accident, Critical Illness/Cancer, Term Life and Dental / Vision Insurance.
Premiums will be paid via bank draft or credit card/debit card. Deductions will be made on the 19th of the month prior to the effective date.
Open enrollment begins 9/24/10. In order to get an 11/1/10 effective date, you must enroll no later than 10/15/10. Enrollments between 10/16/10 and 11/15/10 will have a 12/1/10 effective date.
Enrollment will be handled via telephone by Innovative Health Insurance partners. To enroll please call the enrollment center at 1-877-870-4447 extension 2, Monday – Friday from 8 AM to 5 PM CST. An enrollment counselor will explain the plans available to you and in most cases will be able to take all of your enrollment information over the phone.
We urge everyone in our associate community to scrutinize and consider these health benefit offerings. Open Enrollment for the health plan will run from September 24th through November 15th.
